A guiding principle of the PCMH-Kids initiative is that patient centered medical homes address the physical, developmental, behavioral, social, emotional, environmental, and oral health needs of children and youth.

Experienced Nurse Care Managers/Care Coordinators (NCM/CC) were invited to apply for NCM/CC faculty positions whose role will be to provide coaching support for new and experienced NCM/CC and assist them with developing skills in supporting patients/families that have complex care needs.
On March 10, 2017, CTC-RI hosted a learning opportunity at the Rhode Island Shiners Imperial Room as part of the quarterly series with a focus on practice innovation and building the medical neighborhood, including expanding services in advanced primary care, and addressing social determinants of health.

Pediatric Providers and Health Plan representatives, along with CTC-RI, have been working together over the last few months to identify a common definition and algorithm around pediatric patients who would be defined as “high risk” and in need of care coordination. At the end of 2016, the group decided that it would be beneficial to see what other practices have been doing around making their
CTC-RI is arranging for NCQA to come to Rhode Island to provide onsite training on the 2017 PCMH standards. “Introduction to PCMH: Foundational Concepts of the Medical Home” will be offered on July 17-18. This program will provide a thorough review of the PCMH 2017 requirements and the documentation needed to support an application for NCQA recognition.
