On March 10, 2017, CTC-RI hosted a learning opportunity at the Rhode Island Shiners Imperial Room as part of the quarterly series with a focus on practice innovation and building the medical neighborhood, including expanding services in advanced primary care, and addressing social determinants of health.

Experienced Nurse Care Managers/Care Coordinators (NCM/CC) were invited to apply for NCM/CC faculty positions whose role will be to provide coaching support for new and experienced NCM/CC and assist them with developing skills in supporting patients/families that have complex care needs.
