Care Coordination First ECHO Learning Session – Entitlement to Eligibility

In May, CTC-RI hosted the first Care Coordination ECHO session on Entitlement to Eligibility with funding and support from the Rhode Island Department of Health. This was the first session of our seven-session series that focuses on strategies to improve care coordination for families with young adults with special healthcare needs transitioning from pediatric to adult care.
