Quarterly Breakfast of Champions Sep 13 2019 - 7:30am

Please plan on attending our Quarterly Breakfast of Champions. This is an important point in time for primary care in Rhode Island and for the CTC and PCMH-Kids program. Your involvement and input is very important.

  • Welcome to New PCMH Kids Practices and Review of Current CTC-RI Initiatives by Pano Yeracaris, MD, MPH, CTC-RI Chief Clinical Strategist

  • Statewide Community Health Team Network as an Extension of Primary Care: Expanding to Serve High-Risk Individuals and Families to include a discussion of connecting high-risk children and families to an expanded CEDAR program by Pano Yeracaris, MD, MPH, CTC-RI Chief Clinical Strategist, and Linda Cabral, MM, CTC-RI SBIRT/CHT Project Manager

The Care Transformation Collaborative of RI (CTC-RI) and Providence Community Health Center are hosting the “Integrating Medication Assisted Treatment and Primary Care Summit” and we would like to invite you to this special event.

We are excited to welcome keynote speaker Colleen T LaBelle, MSN, RN-BC, CARN from Boston Medical Center, and Program Director for the State Technical Assistance Treatment Expansion Office Based Opioid Treatment. She will be discussing lessons learned and opportunities for primary care enhancements in delivering medication assisted treatment for addiction disorders.

Update on controlling high blood pressure measure

  • 09 Jan 2019

Adult CTC practices will start reporting on the HEDIS 2019 controlling high blood pressure measure (blood pressure control defined as 140/90 for adults age 18-85) beginning 1/15/19 for timeframe 1/1/18-12/31/18. The previous measure (control defined as 140/90 for adults 18-59 or patient with diabetes 18-85; 150/90 for patients 60-85) has been retired effective 12/1/18.
