Twelve months ago, seven primary care practices joined our second Health Transitions of Care learning collaborative to improve transitions from pediatric to adult health care. In May, they presented at the final quarterly meeting to share their experience, patient success stories, discuss sustainability, and next steps.
In May, CTC-RI hosted the first Care Coordination ECHO session on Entitlement to Eligibility with funding and support from the Rhode Island Department of Health. This was the first session of our seven-session series that focuses on strategies to improve care coordination for families with young adults with special healthcare needs transitioning from pediatric to adult care.
The May Clinical Strategy Committee focused on investing in communities featuring a presentation from Nationwide Children's Hospital.
CTC-RI is excited to invite a new cohort of Nurse Care Managers / Care Coordinators (NCM/CC) to participate in the fall 2023 NCM/CC Standardized Core Curriculum G-learn Program. Now in its sixth year, the program was developed in partnership with Geisinger to meet the primary care community’s requests for more care management training in Rhode Island.
CTC-RI launched a new Project ECHO® Program series in March: “Approaches to Dementia Care: Building Blocks for Becoming Age-Friendly.” Our first session, “Better Patient Care with an Age-Friendly Approach,” presented by Ana Tuya Fulton, MD, FACP, AGSF, provided the 30+ participants with strategies to help learners deliver care aligned with the age-friendly approach.
