Care Transformation Collaborative of Rhode Island (CTC-RI) Selection Committee Announces the Appointment of Nurse Care Manager/Care Coordinator (NCM/CC) Faculty Positions
Experienced Nurse Care Managers/Care Coordinators (NCM/CC) were invited to apply for NCM/CC faculty positions whose role will be to provide coaching support for new and experienced NCM/CC and assist them with developing skills in supporting patients/families that have complex care needs.
