ACP SAN HVCC Training Pilot Program Kickoff

ACP’s High-Value Care Coordination (HVCC) Assistance Program aims to help create a more effective, efficient, and patient-centered referral and care coordination experience in practices.
Kickoff meeting and program orientation:
- Light dinner provided starting 5:00pm
Who Should Attend: Practice administrator, clinical leads, ACO key contacts, any staff that you may want to have engage as a train-the-trainer, and practice quality improvement managers are encouraged to attend in-person.
What You Can Expect: For participants not able to join the live program, a livecast will be available through WebEx using the following link during the same time: https://acpmeetings.webex.com/acpmeetings/j.php?MTID=m9897a5b8a1e005c42170e4e21b404661 The program will also be recorded and the playback made available afterward for those who cannot join in-person.
