Care Coordination ECHO Learning Session #2 – Maximizing Autonomy: Guardianship and Less Restrictive Alternatives

In June, CTC-RI, with funding and support from RIDOH, hosted an ECHO session on Maximizing Autonomy: Guardianship and Less Restrictive Alternatives. This is the second session of our seven-session series that focuses on strategies to improve care coordination for families with young adults, with special healthcare needs, transitioning from pediatric to adult care. Our presenter, Jeannine Casselman, JD, provided participants with information on how to obtain guardianship of a person in the limited circumstances where they are unable to make any decisions for themselves, identify less restrictive tools that can be used to preserve autonomy, and apply role-aligned and caregiving problem-solving strategies in partnership with individuals, families, and communities. Kathleen Kuiper, BS, CCHW, lead a rich discussion combined with a case presentation that had a focus on family and patient voice. 42 participants attended live to share their experiences and ask questions.
