CTC-RI Selects Six Teams for the Rhode to Equity Initiative

The Rhode to Equity is an innovative funding opportunity made available through the RI Executive Office of Health and Human Services (RI-EOHHS) Health Systems Transformation Project (HSTP). The intent of this 12-month learning and action collaborative is to provide 6 cross-sector teams, with a Health Equity Zone (HEZ) as the project lead, the opportunity to test and evaluate strategies that will build leadership and operational capacity for clinical-community linkages, and enhance place-based initiative’s ability to improve both health and social outcomes.

CTC-RI is pleased to announce the 6 selected teams, led by the following HEZs:

Central Providence HEZ Pawtucket/Central Falls HEZ  02907 HEZ
East Providence HEZ Washington County HEZ  Woonsocket HEZ

 

Selected teams will obtain customized technical assistance from national health equity content experts and be supported in applying well-designed, evidence-based population health tools. Through this 12-month learning and action collaborative, they will also have the opportunity to engage with peers in learning sessions to explore “cross pollination” of ideas and understanding. By the end of the year, these teams will be well-positioned to scale their work to further advance community and clinical solutions and policy.

The 6 selected teams, along with their identified team partners (HEZ, accountable entity leadership, primary care local clinic, community health teams, community-based organization and person with lived experience) will participate in a 2-part kickoff session on July 14 and July 29 facilitated by content expert Somava Saha, MD, MS, Well Being and Equity in the World.
