RIDOH and CTC-RI Host Rhode Island Diabetes Health Equity Challenge 2020 Learning Session

The Rhode Island Department of Health, together with CTC-RI, recently hosted a learning session "Learning from the Person; Planning for the Population" for Rhode Island stakeholders and the two selected Health Equity Challenge (HEC) Teams (representing Pawtucket and East Providence). Somava Saha MD, MS, Executive Director Lead from Well Being in the Nation facilitated the discussion on learning from people to meet the priority needs of the population using the process and purpose of population segmentation based on risk. The two HEC teams used information gained from the interviews they conducted with people with diabetes from their communities to brainstorm strategies for addressing population needs. This program is part of a five-month challenge to improve health and social outcomes for people living with diabetes by addressing systemic inequities in the context of COVID-19.
