Nurse care managers learn skills for having advance care planning conversations in primary care

Connie Ducharme, RN, BSN, CHPN from Integra ACO hosted a learning session at the March NCM meeting on "Advance Care Planning at Primary Care Practices: Using tools from the Conversation Project to increase skills." Resources, such as the Conversation Starter Kit and Serious Illness Conversation Guide were shared, and ways to increase advance care planning in their practices were discussed. As part of the Advance Directive Learning Network program, CTC will be hosting an advance directive learning network meeting on April 18 from 7:30-9:00am at Rhode Island Quality Institute (50 Holden Street, Providence). Susan Whittaker, CPC, CPMA from Healthcentric Advisors will be presenting on "Medicare's Annual Wellness Visits: A Gateway to Quality." The program will also provide information on how to code and bill for advance care planning conversations as part of the Medicare Annual Wellness visit.
