Healthy Tomorrows Cohort 3 Final and Cohort 4 Kickoff Learning Collaborative Meeting

Supporting family access to well-child health care and family supports through primary care and family visiting partnership.

In February, Cohort 4 practices and family visiting agencies were introduced to the opportunities of the Healthy Tomorrows program through Cohort 3’s final reportout of their successes, challenges and recommendations. They shared heartwarming patient stories of successful collaborations between primary care practices and family visiting agencies that resulted in positive outcomes for patients and families. Sara Remington, RIDOH, presented family visiting metrics from participating family visiting agencies that demonstrated consistently higher performance than RI statewide averages. Additionally, survey results revealed greatly improved communications, teamwork and care coordination between family visiting and primary care practices. Tiffaine Cataldo, a member of the Healthy Tomorrows planning team representing the family voice, shared her experience with the Healthy Tomorrows program. In the next 3 months, Cohort 4 practices and family visiting agencies will begin building relationships through mutual awareness. The next learning collaborative meeting for Cohort 4  will be May 20. Special thanks to Tufts Health Plan and RIDOH who are providing the funding for this important quality improvement initiative.
