CTC Quality Measures and Best Practice Sharing

Adult and Pediatric CTC practices reported out on their clinical performance using the updated measurement specifications. Practice Reporting co-chairs, Andrea Galgay and Patty Kelly-Flis, reviewed changes to the measurement specification document. Practices are encouraged to view practice quality performance information which can be found in the secure area of the CTC portal (Business Intelligence (BI) Links to View Practice Quality Data). Using these BI links, practices can determine if performance meets thresholds, and "patients to goal" if performance is less than the target. 

Practice Transformation co-chairs, Charlotte Crist and Sarah Fessler, facilitated the "best practice" sharing discussions by practices that have demonstrated top performance on the new adult quality measures. Emily Xavier from Providence Community Health Center and Aimee Schayer from Coventry Primary Care discussed the work their practices have done to improve eye exam rates for diabetic patients. Corrine Robinson from CCAP provided insight on how her organization improved colorectal cancer screening rates
