Pediatric High Risk Sub-Group Updates

Pediatric Providers and Health Plan representatives, along with CTC-RI, have been working together over the last few months to identify a common definition and algorithm around pediatric patients who would be defined as “high risk” and in need of care coordination.  At the end of 2016, the group decided that it would be beneficial to see what other practices have been doing around making their high risk lists more reflective of the need within the pediatric population.

On January 26, 2017, Ed McGookin, MD, FAAP, from Coastal Medical and Linda Hughes, BSW, MA, from East Greenwich Pediatrics presented on what their practices are doing to identify their high risk patients.

The next Pediatric High Risk Sub-Group webinar will be held on February 21, 2017 @ 12pm, when Dr. David Keller will provide information via a webinar on the Pediatric High Risk model that he is using in Colorado.  This webinar will be very informative as it additionally provides information on how they have been able to identify risk using social determinants of health as well as claims data and other practice based information.
