21 practice sites successfully completed the 12-month collaborative, “Using Technology to Improve Care for Patients with Chronic Conditions”.  There were 9 adult, 3 family medicine and 9 pediatric practices who expanded the use of technology to help patients better manage their chronic conditions.

Medicaid Pediatric Healthcare Recovery Program Behavioral Health Technical Assistance Learning Sessions

May’s Behavioral Health Technical Assistance learning session focused on Behavior Plan Basics, where participants learned about the core elements of effective behavior plans as well as common pitfalls and why behavior plans fail. Sample behavior plans and behavior contracts that support healthy routines in older children were shared (e.g. sleep, managing electronics).
